YMCAs are critical partners in improving individual and community health. Beyond a wide range of exercise classes, our Ys provide expert instruction and a supportive community environment that helps participants make manageable lifestyle changes. Community Health Initiative
The NC Alliance of YMCAs fosters collaboration between our Ys and local health care providers to help improve the health of North Carolinians.
As a state, we are moving towards bridging the gap in care and improving quality of life for chronically ill patients and those seeking to prevent chronic diseases.
YMCAs are pioneering an innovative Medical Membership model where three services are bundled together – a YMCA membership, participation in an evidence-based health program, and care coordination.
With a physician referral, individuals and families are referred to the YMCA as a result of a health diagnosis, such as pre-diabetes, high blood pressure, weight management or cancer. YMCA of the Triangle is piloting this model, with the goal to expand this membership model statewide.
Evidence-based Health Programs
The Y’s chronic disease prevention programs, led by certified health coaches, have proven successful in guiding people to make the lifestyle changes needed to improve health. More than 9100+ in NC have improved their health through Y programs.
YMCAs are partnering with hospitals and health systems, physician practices, accountable care organizations (ACOs), and health insurers to increase access to the Y's evidence-based programs to more people across the state.
